THE CONTRACT NEEDS TO BE COMPLETED ONCE PER SCHOOL YEAR, PRIOR TO PARTICIPATION IN THE STUDENT-ATHLETES FIRST SEASON OF PLAY IN THE SCHOOL YEAR. THESE CONTRACTS WILL BE KEPT WITH THE SCHOOL’S ATHLETIC DIRECTOR.
